- Instructions for filling out forms: Print out the Non-covered Procedures and HIPAA Notice of Privacy forms and sign where indicated. Fill out the Information Sheet and Patient History forms online, then print out and sign the bottom of the Patient History form. Bring all four forms with you to your next appointment.
